$950 per team. $400 deposit due by April 30, 2010

**Tournament fee $850.00 if paid in full by April 1, 2010**

Final balance due May 15, 2010. Multiple team discounts available, e-mail for details.

Registration fee includes 5 guaranteed games per team, Meltdown 2010 T-Shirts for all coaches and participants and player entry tickets into the Meltdown Skills Competition.

U-13 participants must be 12 or younger as of 12/31/2009.
U-15 participants must be 14 or younger as of 12/31/2009.
Roster Agreement

All participants must be a registered US Lacrosse member and complete Waiver Form. Coaches must also complete Signature Form to verify all player's US Lacrosse membership.
